Employment Bank Annexure - III Submission Guidelines

Those candidates who are receiving grants of Unemployment Assistant (Rs. 1500/- per month) from Government of West Bengal are required to submit an affidavit containing the declaration of utilization of the Grant/Assistant/Money for the development of the recipient to improve employment ability of the recipient. This self declaration affidavit is called Annexure - III of Employment Bank. Every recipient of the Yuva Utsaha Prakalpa (YUP) or  Bekar Vata has to be submit the Annexure - 3 in an interval of six months (ie. 2 times in a year). if you do not submit you may not receive grant further.

How to Download Annexure III of WB Employment Bank ?

The Annexure - III of Employment Bank has been changed a little bit than previous one. You should Download the Revised/latest version of Annexure Three from EB Website or Click here to download Employment Bank Annexure - III.

ID, Write down Your Name, Your Father's Name, Write Course Details in Box provided in the form (Name of Course, Name & Address of the Institute, Date of Starting of the Course, Remarks if any, do signature in full, put your name in capital letter in the section "name of the beneficiary, write date & place. Now your Annexture III for Employment Bank has been filled up and you may Submit The form along with other documents (if necessary).

 

   

How to Submit Annexure III of Employment Bank of West Bengal ?

Be ready with filled in Annexure - III and other documents (if necessary) and then submit form to the SDO office or Joint Director of Employment, Kolkata. Candidates of District Area are required to submit Annexure - III to SDO (Office of Sub-Divisional Officer of the District), candidates of Kolkata area are required to submit Annexure - III to Joint Director of Employment, Kolkata.

Submission of Annexure -III for List-1 candidates

• Beneficiaries under LIST-1 of YUVASREE are requested to submit Annexure-III to SDO office/Saltlake Purta Bhavan (for Kolkata applicants) within 19.05.2014 to 09.06.2014

• Following items will be considered as 'Courses / Training' for submission of Annexure-III:

• Pursuing General Education [like MP/HS/Graduation etc] during the period of assistance from any Government / Private Institution / Board / Council etc;

• Pursuing Professional/Technical Education or Training [like ITI, Polytechnic Diploma, B.Tech, B.Ed, MBA etc] during the period of assistance from any Government / Private Institution ;

• Pursuing Vocational Education or Training of any level during the period of assistance from any Government / Private Institution;

• Undergoing Coaching for job-oriented competitive examinations during the period of assistance from any Institution.
